A federal monitor who is overseeing the New York City Police Department says officers have been failing to document encounters with people they stop and question on the street. The monitor was appointed after the NYPD’s stop-and-frisk tactics were found unconstitutional. The report calls into question a recent drop in the official tally of stops.
